U.S. Passports
Application Procedures


Passport applications are accepted, in person, from 9:00 a.m. to noon and from 2:00 - 4:00 p.m. Monday - Friday. If you have an emergency need for passport services outside of these hours, please see the contact information page to set up a special appointment.

IF YOU ARE RENEWING YOUR PASSPORT WHICH EXPIRED LESS THAN 15 YEARS AGO AND YOU ARE OVER AGE 16,

Submit:
1. Proof of Citizenship and Identity (your U.S. passport)
2. Completed Application Form DS-82 You can fill this form out on-line at http://travel.state.gov/passport/forms/ds82/ds82_843.html
3. Two identical photographs, taken within the last six months, 2"x2" with dimensions as shown on the Application Form;
4. $75 in US dollars or the equivalent amount in Serbian Dinars.

IF THIS IS YOUR FIRST PASSPORT, IF YOU ARE UNDER AGE 16, IF YOUR MOST RECENT PASSPORT WAS ISSUED MORE THAN 15 YEARS AGO, OR YOUR PASSPORT WAS STOLEN OR LOST

Submit:
1. Proof of Citizenship and Identity:
2. Completed Application Form DS-11, available for downloading at: http://travel.state.gov/passport/forms/ds11/ds11_842.html;
3. Two identical photographs, taken within the last six months, 2"x2" with dimensions as shown on the Application Form;
4. $100 in US dollars or the equivalent amount in Serbian Dinars; for children under the age of 16 is $85.

SPECIAL REQUIREMENTS FOR CHILDREN UNDER 16:

Children under 16 must appear in person and we are required to see evidence that both parents consent to the issuance of a passport. Please review the following information for detailed instructions: http://www.travel.state.gov/passport/get/minors/minors_834.html#

STOLEN/LOST PASSPORTS:

If your passport was lost or stolen, you must also complete form DS-64 http://www.travel.state.gov/pdf/DS-0064.pdf. You should report the theft of a U.S. passport to local police and notify the Embassy. Please bring a copy of the police report with you when you visit the Embassy.

AMENDING A PASSPORT:


Effective September 26, 2005, as an additional security enhancement, we can no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error will apply for replacement passports.

If the change is within one year after issuance, please submit a U.S. Passport Re-Application Form (DS 5504), available at http://travel.state.gov/passport/forms/ds5504/ds5504_2663.html, together with the passport that needs the change, the documentation required, and new passport pictures. There will be no charge for routine processing.

If the change is more than one year after issuance, please submit an Application for Passport By Mail (DS-82) together with the passport that needs the change, the documentation required, and new pictures. The cost includes the $55 application fee and $20 security surcharge, for a total of $75 for routine processing. Of course, if the requested data change is due to a printing error by Passport Services, there will be no charge if the amendment is made within one year since passport issuance.

Those only needing to add visa pages to their passports will submit an Application for Additional Visa Pages (DS-4085), available at http://travel.state.gov/passport/forms/ds4085/ds4085_2662.html. This service is always free of charge when routine.

All of these forms are available online at travel.state.gov. The Passport Amendment/Validation Form (DS-19), previously used to amend passports, will no longer be valid.


PASSPORT PHOTOGRAPHS:

To apply for a new passport you must submit two identical 2" x 2" (5 x 5 cm), light background photographs. The photos must be recent - taken within the last six months. Although you may use the photographer of your choice, this size of photograph is not common in Serbia. Be sure to mention that you need photographs for an American passport and show the photographer the specifications on the passport application form.
